Print these forms out and distribute to your members attending nationals ASAP.
In order to complete the registration,
Athletes Must Upload:
1) Passport-Style Photo
2) Valid Passport OR Birth Certificate & Photo ID Combo
3) Signed Anti-Doping Form
4) Signed Athlete Concussion Protocol Form
5) Signed Code of Conduct
6) Signed Pre-Competition Medical Form
7) If Under 18, Signed Parental Consent Form
Coaches Must Upload:
1) Passport-Style Photo
2) Signed Code of Conduct
3) Signed Coaches Concussion Protocol
4) Hard Copy of a Vulnerable Sector Check or Enhanced Police Check dated no earlier than September 1, 2018

Support Staff Must Upload
1) Passport-Style Photo

Registering For Nationals
Individuals can register themselves, as long as they are in your membership database. If they are not, they cannot register for nationals.
– Individuals will look up their membership number, using their name, club and birth year
– Once selected, the required info should populate from the membership database
– They will have to upload all the required files
– They will have to pay online at the time of registration
